The successful candidate will be responsible for:

? Conduct assessments including woman?s strengths, needs and risk factors to determine suitability for woman abuse services and produce recommendations with respect to what other services may be necessary

? Facilitate case management coordination which includes the provision of information, advocacy and referral to both agency and outside resources; including shelter and legal matters

? Provide crisis response and safety planning

? Provide individual counselling and group counselling to survivors of intimate partner abuse

? May provide support and coordination to the ASTEH shelter

? Will also provide support and services to other agency programs as required

? Ensure accurate and timely data collection for internal and external record keeping

? May be involved in program development and community liaison activities

? Ability to work effectively in a multi-disciplinary team

Qualifications:

? BSW or MSW

? Minimum 3 years clinical experience

? Member of OCSWSSW in good standing

? Knowledge and exposure in CBT and group psychotherapy considered an asset

HOSPICE SOCIAL WORKER

? Provision of assessment, effective counselling support, advocacy and/or direct services to Hospice clients and their families

? Extensive use of crisis intervention, as well as significant use of individual and family counselling

? Participates in interdisciplinary collaboration with partners in the Hospice program

? Participates on Hospice Team Community Rounds

? Advocacy with community agencies such as Circle of Care to ensure necessary provision of services

? Provides concrete linkages to community healthcare resources

? Participates and monitors the evaluation of the program, submitting reports and working with the Committee of the Hospice Program

Job Profile:

The Mental Health Worker facilitates and supports recovery for adults facing mental health challenges through empowerment and respect by providing a range of social, educational, and recreational services. Working collaboratively with members, other service providers, and the Recovery Team, the Mental Health Worker assesses plans, develops, coordinates, and provides rehabilitation and support.

Using a recovery-based approach, the Mental Health Worker assists members to develop and use interpersonal, social, and leadership skills that will optimize their strengths and improve their quality of life, and mental and physical health and well being. The Mental Health Worker collaborates with a range of formal and informal services, refers members to external resources, and may also participate in case conferences and systemic initiatives to improve availability, accessibility, and equity of resources.

From time to time, the Mental Health Worker may be required to handle conflicts in the Program, and intervene in crisis situations.

In addition to compliance with all organizational policies and procedures, the incumbent is also expected to behave ethically, demonstrate competence in effective communication and team building, demonstrate effectiveness in working in a collaborative environment, and demonstrate confidence in decision-making and in building relationships.

This position reports to and is supervised by the Program Manager.

Requirements:

? Degree or diploma in health or social services

? Degree plus four (4) years experience in a community-based organization working with adults with serious mental illness

? Diploma plus six (6) years or more experience in a community-based organization, working with adults with serious mental illness

? Current registration with the OCSWSSW or a regulatory body under the Regulated Health Professions Act

? Education and experience related to supportive counseling, community development, crisis prevention, advocacy, multicultural issues and consumer empowerment is preferred

? Knowledge of systemic issues such as poverty, unemployment, stigma and the isolation felt by individuals with serious mental illness and their families

? Knowledge of the Ontario Mental Health Act, mental health reform principles, the Substitute Decisions Act, and the Health Care Consent Act and PHIPA requirements

? Knowledge of PSR and Recovery as guiding principles

? Ability to apply Recovery principles and empowerment oriented philosophies and practices in work with consumers

? A second language reflecting the local community is an asset

? A valid Canadian driver?s license, a minimum $1,000,000 third party liability insurance and proof that insurance covers use of personal vehicle to transport clients

Hospice Palliative Care Home Visiting & Bereavement Support Volunteers

Training begins February 2013

Caring, compassionate and reliable volunteers are needed to provide non-medical care to people facing life threatening illness, death and bereavement.

You can volunteer as:

?

  • A hospice palliative care home visiting volunteer* providing emotional, social, spiritual and practical (non-medical) support for people living with a life-threatening illness, their caregivers and families.
  • A bereavement peer facilitator** providing one to one support and/or co-facilitating mutual support bereavement groups for people grieving the loss of someone significant to them.

We know that with the right support, services and information, caring for someone at the end of life can be manageable and meaningful for all involved. We also believe that it is possible to learn to live with grief ? a natural response to loss ? if given safe and supportive opportunities to explore that grief in ways that make sense to you.

Job Description

Under the supervision of the chef, he/she will assist in a variety of tasks related to receiving, storing, preparing, cooking and serving of food including the general housekeeping of the food service areas and equipment.

Position Responsibilities and Accountabilities

1) Assist in housekeeping duties such as, but not restricted to:

-??????? Washing and disinfecting pots, pans, tables, and various containers and other utensils.

Position Summary:

Funded by the Ministry of Children & Youth Services, Youth Court Counsellors work with duty and defense counsel to develop community-based program and action plans sensitive to the principles of the YCJA and that will increase opportunities for diversion and pre-trial releases for young persons in the youth criminal justice system.

The successful applicant may be required to work without dedicated office space, with limited privacy as well as provide coverage at sites in Toronto and possibly Newmarket.

Key Responsibilities:

  • Counselling/Case Management ? Assess clients? needs, risk, and program participation.? Develop effective client specific plans and court reports. Monitor youth and progress of their client specific plans where applicable.
  • Community Liaison/Outreach ? Networking, advocating, and educating community partners and service providers in order to identify suitable community services and resources that are accessible and responsive to client needs.
  • Community Development ? Assist in building supportive relationships with Officers of the Court, community resources, public officials, partnership agencies, and members of the public on program objectives.
  • Administrative ? Prepare progress, court and statistical reports as required

Required Qualifications:

  • Post Secondary diploma/degree in the social services field plus
  • Minimum of two (2) years of related experience or equivalent in: developing effective client specific plans and court reports, counselling, case management, risk assessment, community outreach and community development, working cooperatively with community service providers in the provision of service to young persons involved in the youth criminal justice system
  • Working knowledge and good understanding of the criminal justice system and social service agencies and the principles of risk, need, and intervention for at-risk/high risk youth.
  • Assessment skills required for both the delivery of service to young persons and daily operational/programming issues, including determination of client suitability and risk.
  • Excellent counselling, program planning, communication, interpersonal, organizational, prioritizing, problem-solving, decision-making, and administrative skills.

Demonstrated use of core competencies in:

  • Information Gathering ? investigate personally and then probe deeply,? asking a series of ever more pointed questions to get to the root of a situation or problem or to uncover a potential but hidden opportunity.

RESPONSIBILITIES

?

Our counsellors provide high quality professional telephone, web and chat-line counselling, education /referral to children and youth contacting Kids Help within a context of protection and empowerment of children and families to support the care of children and adolescents. The successful incumbent will provide assessment, counselling and support to children and youth in crisis and where appropriate provide referral to local community services to ensure effective professional response to the specific crisis. This includes assisting children and youth in distress and/or requesting supportive and educational services using methods and interventions defined within the context of professional social work practice while incorporating additional skills generally gained through exposure to other ?helping professional practices and methods?, and completing required documentation to support direct service for? telephone,? Web, or? chat-line? interventions. Our Counsellors also attend and participate in case conferences, case supervision and debriefing.

Our counsellors also:

  • Participate and attend in-service training, clinical consultations, monthly staff meetings and other professional development activities
  • Maintain a sound knowledge of various national and provincial laws affecting the lives of children and young people at risk to provide sound decision making support regarding appropriate and relevant referral to community authorities
  • Share areas of expertise and participate in developing resources for the department
  • Follow professional problem solving models
  • Actively participate in the process of individual clinical supervision and coaching
  • Facilitate the growth and understanding of the importance of a healthy lifestyle in which each child or youth is equipped with various tools and skills required to respond to the day-to-day demands of living in a dynamic family group, within a community and within a larger society

CANDIDATE REQUIREMENTS

  • Degree or diploma in applied social service
  • Fluency in French
  • Minimum three years of relevant? experience in a social service sector
  • Excellent communication skills, with the ability to create and maintain trusting relationships with kids from 5-20 years of age
  • The ability to engage clients in non-judgemental problem solving
  • The ability to analyze situations, make appropriate decisions concerning clients? needs and provide appropriate therapeutic service
  • Excellent written? communication skills and the ability to use computer applications including posting and on-line chat software
  • Experience within a national non-profit environment and in collaborating across regional contexts within a multi-stakeholder environment

Job Description

This person will work closely with our Market Coordinator and Food Advocate to support the success of LEF?s weekly Good Food Market. This role will also involve working on issues related to food security that are prevalent in our community.

The Market/Food Animator position will include the following tasks:

Work with volunteers and staff to support our weekly food market, including set-up, teardown and selling of produce.

  • Customer service at the market.
  • Liaison and outreach with vendors that participate in the market.
  • Help with weekly ordering for the market.
  • Complete weekly tracking forms.
  • Build a volunteer outreach, engagement, and appreciation strategy.
  • Support and train new volunteers in market roles.
  • Help shape market directions through evaluation of the market, focus groups with market patrons, and observations at each market.

  • Promote the access and availability of health food options.

Nature & Scope:

The West End YMCA provides a variety of health, fitness, recreation, and community programs to more than 10,800 members, including children, youth and adults, who represent the diversity of Toronto and its communities.

Reporting to the Health and Fitness Programs Senior Director the Health and Fitness Programs Supervisor (Group Fitness) provides direct leadership to execute all group fitness activities in the facility including group fitness programs and volunteer development in group fitness.? The Supervisor provides a superior experience to our members in General Membership areas. Implementing our Member Insight Strategy and using our Membership Involvement Model, identifies initiatives to increase Membership on Roll, Retention, Satisfaction, and Philanthropic contributions in our Group Fitness Programs.? The Supervisor must ensure that all members of this team are proactive in their ability to stay abreast of leading trends in health, fitness and demonstrate exceptional relationship building skills. The recruitment, development, certification, retention and deployment of a team of volunteers to work in this area is also a major focus of this department. As well as increased class offerings.

Responsibilities:

Develops, implements, monitors and evaluates programs within the Association?s Membership Services Plan, which will attract and retain the diverse membership of the centre.

Provides direct program leadership to a variety of group program areas including group fitness classes and small group training including TRX, Aqua-Fit and Recreational Sports for the equivalent of 60-70% of scheduled shifts.

Provides direct leadership as a YMCA Trainer in Fitness in delivering the YMCA Fitness Leader Certification and works towards the YMCA Training Educator Level.

Assists Health and Fitness Programs Senior Director with recruiting, supervising and developing effective staff and volunteer teams within the department. Maintains records for volunteers using Raiser?s Edge.

Assists Health and Fitness Programs Senior Director in development and management of all performance indicators and standards including those related to group fitness, retention and YMCA Priority Sam standards.

Provides leadership and support as a member of the Centre?s senior staff team. Acts as an effective Service Leader.

Participates in task groups, special assignments and opportunities that further the Association mission, vision and strategic initiatives including the Core Service Functional Group.

Qualifications:

  • YMCA Trainer certification an asset
